Adults

Each family member over 18 must submit a completed Adult Intake Form online or in hard-copy.

Children

For all children ages 3 to 12, parents or guardians must submit the following.

  • Minor Intake Form
  • Consent for the Treatment of Minors

Teenagers

For all teens ages 13 to 17, the adolescent must submit the following.

  • Adolescent Intake Form (to be completed by the adolescent)
